At the Ohio Center for Cosmetic Dentistry our purpose is to fulfill a twofold promise to our patients. Our primary concern is to treat every patient with the same professionalism, kindness and respect we would a family member. There is nothing more important than keeping our patients comfortable during and after treatment, and earning their confidence. Regardless of how finely tuned our technical skills may be, we are fully aware that our patients will not care how much we know, until they know they know how much we care.

Secondly, we commit ourselves to providing to our patients the most current concepts in cosmetic dentistry. We have often observed how changing a patient's smile will enhance their self-image. We are more than improving smiles, we are improving lives. It is crucial, therefore, that we at the Ohio Center for Cosmetic Dentistry constantly continue to hone our skills and stay apprised of up and coming technology so as to remain on the cutting edge of modern dentistry.

• Visual relaxation with cable television. Choose from over 60 channels

• One-on-one personalized care

• Relaxed and never-rushed atmosphere

• "Pause button" attached to each chair to allow you to audibly signal when you need to take a break during treatment.

Our office strictly follows the safety and sterilization guidelines recommended by the American Dental Association, Ohio State Dental Board and OSHA. We constantly upgrade our infection control techniques by attending frequent courses and holding regular staff meetings so as to insure your safety and peace of mind. Some of the techniques we employ include:

• The use of gloves, masks, and protective eyewear

• Sterilization of all instruments placed intra-orally in a autoclave

• Consistent use of a contracted sterilization monitoring service to insure the effectiveness of equipment

• Disposal of all non-sterilizeable products to prevent cross-contamination

• Sterilization of all dental handpieces

• Covering patient chairs, x-ray units, and light handles with disposable plastic wraps

• Filtered water supplied to dental handpieces and air/water syringes.
  

At the Ohio Center for Cosmetic Dentistry we are committed to providing for our patients the most caring and highest quality of service possible. We take our responsibility to our patients seriously and expect our patients to respect our office policies in return.

Method of Payment:

Unless specific prior arrangements have been made, full payment is due at the time of service. We will accept payment in cash, checks, Visa, Mastercard, American Express or Discover Card.

Payment Plans:

For extensive treatment (over $250.00), we offer interest free payment plans for either 6 or 12 months. This payment plan is subject to prior credit approval. A credit application can be filled out at the office and faxed to the finance company immediately. Within 10 minutes our office will be informed if the application has been approved.

Dental Insurance:

At your first visit please bring with you a completed insurance claim form so that we may enter the appropriate information into your computerized patient file. At subsequent visits insurance forms will no longer be necessary unless otherwise requested. After your second visit, we may accept assignment of your insurance benefits. We will estimate your portion of fees which will be due at the time of service.

In most of the extensive treatment cases, a pretreatment estimate is submitted to your insurance company. This will eliminate any surprises or disappointments as to what your particular insurance company's benefits may be. Once we are aware of prospective insurance benefits, your portion of payment is due in full at the start of treatment. In some cases, the benefits paid by your insurance carrier are not what were anticipated. The balance of your bill is your responsibility whether your insurance carrier pays its expected portion or not.

Please note that your insurance policy is a contract between you and your insurance company. However, we will work with you to provide your insurance company with all of the necessary information to pay their rightful portion in a timely manner. The courtesy of completing and submitting your insurance forms to your carrier will be extended to all of our patients. We only ask that you keep us updated as to any changes in your insurance coverage.

At the Ohio Center for Cosmetic Dentistry we are fully aware of the hectic demands on your time. In light of this, we have structured our office hours to enable you to schedule an appointment at a convenient time.

We know that occasionally circumstances may occur when you cannot keep a scheduled appointment. We kindly request that you give us 24 hours notice so that we may fill your appointment time.
